ازاي بنختار دواء الاكتئاب المناسب للحالة؟

 

How to select the best antidepressant medications ?

أو

حاجات لازم الطبيب يخلي باله منها وهو بيختار الدوا الفلاني ؟

أو

ليه الدكتور اختار الدواء الفلاني للمريض الفلاني ؟

أو

مين الأدوية اللي متنفعش مع مرضي معينين ؟

بداية لازم نكون عارفين ميزة وعيب كل دواء فيهم.

 

1.مريض مش عاوز يزيد في الوزن او يحافظ علي وزنه :

Patient who is trying to avoid weight gain :

* Avoid mirtazapine

بلاش mirtazapine

طب نستخدم مين ؟

Consider : Bupropion

Bupropion غالبا يصاحبه نقص في الوزن

 

2.مريض عنده مشاكل في القلب :

Patient with cardiac issues (Post MI, bradicardia, heart failure "HF" )

بلاش citalopram &escitalopram 

طب نستخدم مين ؟

Consider: sertraline (preferred)

 

3.مريض عنده ريسك من seizures :

Patient at risk of seizures (seizures disorder , recent alcohol or sedative withdrawal) or history of anorexia / bulimia :

*Bupropion is contradicted 

ممنوع نستخدم Bupropion

مش بس كده

؟؟؟؟

Bupropion doses should not exceed 450mg day in any patient due to risk of seizures.

مينفعش نزود او الجرعة لأي مريض بياخد Bupropion عن 450 مجم في اليوم لانه بيزيد من خطورة حدوث seizures.

 

4.مريض اكتئاب وعنده ألم 

Patient with depression and pain (neuropathic or musculoskeletal) Duloxetine is indicated for both.

يفضل استخدام Duloxetine

 

5.مريض عنده أرق و وزنه قليل 

Patient with insomnia and low body weight (i.e., weight gain desirable) 

ممكن نستخدم Mirtazapine

*Mirtazapine side effects may be beneficial (sedation and weight gain)

 

6.مريض مش عاوز يحصله sexual dysfunction 

Patient who is trying to avoid sexual dysfunction 

نتجنب SSRIs and SNRIs

Avoid : SSRIs and SNRIs(high risk)

طب ممكن نستخدم مين ؟؟؟؟؟

Bupropion or mirtazapine

Consider: bupropion and mirtazapine (less sexual dysfunction)
