ان شاء الله هنتكلم اليوم عن الصفرا كان فيه كثير طلبها مني واتأخرت شوية في كتابتها لكن الحمد لله اتكتبت يارب تفيدكم 😍😍😍.

الصفرا ممكن تكون في الدم وتظهر علي الجسم لو عدت

2 mg/dl

أقل من كده لا تظهر علي الجسم وتسمي

Subclinical jundice.

Jaundice becomes clinically evident when serum bilirubin level exceeds 2.0 mg/dl.

بداية كرات الدم الحمراء بتتكسر في مكانين :

المكان الأول وده بيمثل ١٥ % من البلوربين الناتج

Ineffective erythropiesis

وده بيحصل في ال BM، نتيجة إن أمهات كرات الدم الحمراء بتتكسر زي حالات

Megaloblastic anaemia, myelofibrosis and Myelodysplasia

Ineffective hematopoiesis may occurs in this disease leads to jundice.

وكمان من

and from myoglobin,cytochromes, and peroxidases.

السبب الثاني وهو الأكبر بالطبع وهو

Destruction of RBCs by reticuloedothelial cells especially spleen

وده بيمثل ٨٥ % من البلوربين الناتج

ايه اللي بيحصل

Hb ➡ degraded into haem and globin.

Globin degraded into A.A. and recycled again

Haem ➡ degradeded into iron and protoporphin

Iron recycled again in the body

Protoporphin converted into ➡ Bileverdin ➡ indirect bilirubin

هنا اتكون ال

indirect bilirubin and it is water insoluble

لا يستطيع السير في الدم ويحمل علي الألبيومين ولذلك لا ينزل في البول وله أسماء

Indirect bilirubin = unconjugated bilirubin = hemobilurbin = Alfa bilirubin

بعد ذلك يذهب للكبد ويحدث ثلاث خطوات مهمة جدا له

1_ uptake of unconjugated bilirubin by liver.

2_ Bilirubin is conjugated with glucuronic acid to form bilirubin monoglucuronide and diglucuronide(conjugated bilirubin); this process is mediated by the enzyme UDP glucuronyl transferase.

Conjugated bilirubin is water soluble.

3_ secretion of Conjugated bilirubin into bile canaliculi ➡ hepatic duct ➡ bile duct ➡ hepatopancreatic ampulla ➡ second part of the duodenum

Conjugated bilirubin is deconjugated by glucouronidase then reduced by intestinal bacteria into stercobilinogen

ال stercobilinogen اما

1_ oxidized into stercobilin ➡ escape into stool and gives it's coloure.

او

2_ absorbed from Porta circulation to the liver and again to the bile in the form of urobilinogen ( enterohepatic circulation).

3_ A small amount of urobilinogen in circulation escapes clearance by the liver and is excreted in urine.
